Hypothalamic disease is a rare cause of thyroid failure. In such instances, circulating T4 levels are low, T3 may be normal or low, and basal TSH is normal or low (Fig. 15.32). The pituitary TSH response to exogenous TRH is usually sluggish and delayed, and values at sixty minutes are higher than at twenty minutes after TRH administration.
